What we review at your first visit
We review your weight history, previous approaches to treatment and current health concerns. I want to understand both your response to earlier treatment and the reasons it became difficult to continue.
You do not need to arrive with a diagnosis or repeat testing simply because you are starting with a new physician. I review the information available and explain any further evaluation I recommend.
Your assessment includes:
- Medical conditions and family history relevant to your weight and metabolic health.
- Current medications, including possible effects on weight or appetite.
- Existing laboratory results and whether additional testing would help guide care.
- Nutrition, physical activity, sleep, emotional wellbeing and daily responsibilities.
- Hormonal or reproductive concerns when relevant to your health history.

How we develop your care plan
- At the second visit, I explain my clinical assessment and review the findings with you. We discuss what they mean for your health and which treatment options fit your needs.
- Your plan may include nutrition and physical activity recommendations, medication when appropriate, and attention to hormonal, metabolic or other contributing concerns. We also consider the practical demands that affect your ability to follow treatment.
- Medication is considered within comprehensive care. SustainMD does not offer a separate GLP-1 program or a medication-only consultation track, and an appointment does not guarantee a prescription.
What you receive after the assessment
You receive an individualized written plan that explains the recommended direction for your care. There is no obligation to join membership afterward.
If you choose Signature Care Membership, I continue working with you through regular follow-up, treatment review and direct communication. The consultation fee is separate from membership and is not credited toward it.
How testing and payment work
My evaluation and interpretation of your results are included in the consultation. Laboratory testing is billed separately. Where covered, you may use insurance for testing; discounted self-pay laboratory options may also be available. Medication and supplement purchases are separate from physician fees.
SustainMD is a direct-pay practice. For payment methods and information about invoices for possible out-of-network reimbursement, see pricing and payment information.
Frequently Asked Questions
Does the consultation include both visits?
Yes. The initial consultation includes both extended physician visits, my interpretation of your lab results and an individualized written plan. Visit length is based on your needs.
Do I need a diagnosis before booking?
No. You can seek an assessment for concerns about your weight or metabolic health without knowing the cause. Your medical history and clinical evaluation guide the next steps.
Can I bring existing lab results?
Yes. Existing results are part of the assessment. I consider their relevance and whether additional testing is needed before recommending further evaluation.
Are lab tests included in the consultation fee?
Lab interpretation is included. Laboratory testing is billed separately, with insurance or self-pay options depending on the test and your coverage. See payment information.
Will I receive a GLP-1 prescription?
A prescription depends on an individual medical assessment. I consider FDA-approved anti-obesity medications, including GLP-1 medications, when appropriate within your broader care plan.
Do I have to join membership afterward?
No. Membership is optional. You can review your plan and discuss whether ongoing care fits your needs before deciding.
Is the consultation fee credited toward membership?
No. The consultation and membership are separate services. The Pricing page explains their fees and inclusions.
What happens after I book?
I will contact you with the next steps and explain what is needed for your assessment.
