SustainMD offers direct physician care with clear fees and a choice of appointment options. You pay the practice directly for professional services, while insurance may still support other parts of your care.

What your physician care costs

Initial Consultation

$500for two extended physician visits.

The consultation includes a comprehensive history and medical assessment, interpretation of your lab results and an individualized written care plan. I conduct both visits. The consultation fee is separate from membership and is not credited toward it.

Signature Care Membership

$200per month for ongoing care after the initial consultation.

Membership includes monthly physician visits, accountability check-ins and direct communication by text and email. Care includes ongoing obesity management, body composition analysis, relevant lab evaluation and medication management. I also handle medication prior authorizations, insurance appeals and prescription concerns when needed.

There is no minimum commitment. A patient may cancel within 30 days if they are not satisfied.

How laboratory costs work

My assessment and interpretation of your lab results are included in the relevant physician service. Laboratory testing is billed separately.

You may use insurance for covered testing or choose discounted self-pay options. Members have access to discounted laboratory pricing through Quest Diagnostics and LabCorp. Coverage and any patient responsibility depend on your insurance and the tests ordered.

How medication and supplement costs work

Pharmacies bill separately for medication. I pursue prior authorizations and appeals when appropriate and discuss available self-pay options if coverage is unavailable. An insurer's decision or a particular medication price cannot be guaranteed.

Supplements are also purchased separately through the supplement dispensary. These purchases are not included in consultation or membership fees.

How insurance may still help

SustainMD does not bill insurance for physician services. However, direct payment for my care does not prevent you from using insurance for covered prescriptions or laboratory testing.

For eligible visits, including one-time appointments, I can provide an itemized invoice for you to submit to your insurer for possible out-of-network reimbursement. Reimbursement depends on your plan, deductible and insurer's rules. Membership fees may be treated differently; confirm eligibility with your insurer before relying on reimbursement.

Credit and debit cards, ACH and Zelle are accepted. FSA and HSA cards are also accepted, subject to the eligibility rules of your employer or plan administrator. Card acceptance does not establish that every service or membership payment is eligible.

Does the consultation fee cover both visits?

Yes. It covers both extended physician visits, lab interpretation and the individualized written plan. Laboratory testing itself is billed separately.

Is the consultation credited toward membership?

No. The consultation and membership are separate services. Their current fees are listed above.

Can I use insurance for lab tests or medication?

You may use insurance where those services are covered. Your plan determines coverage, deductibles and other patient costs.

Can I request an invoice for insurance reimbursement?

Yes. An itemized invoice can be provided for eligible visits so you can request out-of-network reimbursement. Payment by your insurer is not guaranteed.

Will insurance reimburse my membership?

Do not assume it will. Membership fees may be treated differently from individual visits. Ask your insurer about your specific benefits before enrolling on that basis.

Can I pay with an HSA or FSA card?

These cards are accepted, but eligibility depends on your plan's rules and the service. Confirm eligibility with your plan administrator.

Is membership required for a one-time visit?

No. You can book a standalone One-Time Visit without membership or the initial consultation.

Is there a minimum membership commitment?

There is no minimum commitment. A patient may cancel within 30 days if they are not satisfied. Review the membership agreement for the applicable terms.
